I, the undersigned, freely enter into the following agreement with Dare Ministries. I have been informed that Dare Ministries is capturing footage and that my name, likeness, image, voice, appearance and/or performance is being recorded and made part of the recording in which I appear, from here on known as the "Product."

1. I grant Dare Ministries and its designees the right to use the Product in any format, now known or later developed. I grant, without limitation, the right to edit, mix or duplicate and use or re-use the Product in whole or in parts as Dare Ministries may elect. Dare Ministries or its designees have complete ownership of the Product, including copyright interests.

2. I grant Dare Ministries and its designees the right to broadcast, exhibit, market and otherwise distribute the Product, in whole or in parts, and alone or with other products, for any purpose Dare Ministries or its designees determine. This grant includes the right to use Product for promoting or publicizing the minsitry and its events.

3. I have the right to enter into this agreement and am not restricted by commitments to third parties.

4. Dare Ministries has no financial commitments or obligations to me as a result of this agreement.

5. I have read, understand, and agree to all of the above.

I, the undersigned, understand that I am participating in the above activities, Volunteering at The Upper Room Coffee House, at my own risk and that Dare Ministries, its Board, and all representatives of the ministry are not liable for any injury or illness that may occur during or as a result this volunteer position and do release and forever discharge and hold harmless Dare Ministries and its successors from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the event I freely and voluntarily participated in held by Dare Ministries.

I understand and acknowledge that this Release discharges Dare Ministries from any liability or claim that I may have against Dare Ministries with respect to bodily injury, personal injury, illness, death, or property damage that may result from the activities I freely choose to participate in with Dare Ministries or occurring while I am participating in said activities.

By signing my name below, I agree to the above liability release and do waive any rights to make any liability or claim against Dare Ministries.

Furthermore, in the event of any injury, I authorize treatment recommended, by a qualified and licensed medical doctor, in the event of any medical emergency which, in the opinion of the attending physician, is necessary. I understand that I am responsible for all medical expenses accrued.
